Schedule

Full-Time, 2pm-10pm

Responsibilities

• Support the health and well-being of residents through ongoing clinical monitoring and support
• Provide assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence
• Work effectively as part of a team with flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition
• Assist in coordination of the resident admission process including screening, assessment, and care needs
• Ensure care is delivered as indicated in resident care plans
• Administer medication and treatment and ensure medications are administered per physicians’ orders
• Assist with coordinating care and services with other outside providers including resident physicians
• Document and report care provided and resident observations including unusual or significant changes
• Maintain a working knowledge of state regulations, policies, and procedures

Qualifications

• Working knowledge of state regulations, policies, and procedures, dictated for compliance
• Graduate from an accredited school of practical nursing
• Current nursing license and state registration (LPN or RN)
• Three (3) years of experience in assessment and geriatric nursing preferred
• A big smile, eagerness to learn, and compassion for our residents
