Wellness Nurse

About the role

The Wellness Nurse at Spectrum Retirement Communities supports the health and well-being of residents through ongoing clinical monitoring and support. They assist in the coordination of the resident admission process, ensure care is delivered according to resident care plans, administer medications and treatments, coordinate care with other providers, document care provided, and report observations.

Responsibilities

• Ongoing clinical monitoring and support for resident health
• Aid in the coordination of the resident admission process
• Ensure care is delivered as indicated in resident care plans
• Administer medications and treatments according to physicians' orders
• Coordinate care and services with other outside providers including resident physicians
• Document and report care provided and resident observations including unusual or significant changes
• Maintain a working knowledge of state regulations, policies, and procedures

Requirements

• Working knowledge of state regulations, policies, and procedures
• Current nursing license and state registration (LPN or RN)
• Three years’ experience in assessment and geriatric nursing, preferred

Qualifications

• Big smile, eagerness to learn, and compassion for our Residents
• Graduate from an accredited school of practical nursing
• Experience in assessment and geriatric nursing (three years preferred)
