About the Role
The Wellness Nurse is responsible for providing nursing triage, health education, and risk-reduction strategies to maximize residents’ health and quality of life through the 8 Dimensions of Wellness.
- Assists residents in achieving health and wellness goals within the 8 Dimensions of Wellness, personal choice, and mind-body-spirit.
- Provides support strategies and referrals for chronic-disease management and acute illness or injury.
- Acts as a resource for options and choices related to supportive strategies to improve health, function, and lifestyle.
- Develops a referral network of service providers in and outside the community with the Director of Resident Services.
- Participates in the collaborative-care weekly meeting and communicates with the multi-disciplinary team to optimize resident outcomes, campus flow, and occupancy.
- Oversees post-acute discharge planning and follow-up support; collaborates with health-center and hospital staff on admissions, transfers, and discharges.
- Attends to medical emergency calls, partners with residents on risk-reduction strategies, and completes incident reports per community protocol.
- Provides annual updates for medical demographic information and advance directives.
- Organizes and facilitates vaccination clinics.
- Encourages utilization of and supports on-site healthcare practitioners.
- Partners with the Community Life Director and Resident Services Director to plan, implement, and evaluate Wellness Programming, health screenings, and health-education opportunities.
- Pro-actively builds relationships with residents to establish trust, respect, and connection.
- Communicates effectively with families and staff; fosters measures to improve community culture.
- Maintains confidential records appropriate for independent living.
- Attends in-service training and educational sessions as assigned.
- Strives to achieve high levels of resident satisfaction using sound judgement and strong organizational skills.
- Performs other duties as assigned by the Resident Services Director.
Requirements
- Active LPN license for the applicable state.
- Three years of experience in a related organization preferred.
- Excellent observation, written, and verbal communication skills.
- Self-directed with the ability to work with little supervision.
- Flexible and cooperative in fulfilling obligations.
- Ability to problem-solve, make ethical recommendations, and define appropriate boundaries.
- Complies with accepted professional standards and practice.
- Basic computer skills with platforms such as Word and Excel.
This position requires background screening through the Florida Care Provider Background Screening Clearinghouse.
