Wellbeing for Life Practitioner

Job Purpose

The Adult Wellbeing Services (AWS) are delivered by a partnership between County Durham and Darlington NHS Trust (CDDFT) and the Pioneering Care Partnership (PCP) and includes the Wellbeing for Life (WBFL) Service. WBFL is a commissioned service in County Durham based on the principle of Your Wellbeing, your way. Based on the model of behaviour change, the team encourages people living and working across County Durham to take small steps to better health in a way tailored to suit them.

About the role

The Wellbeing Practitioner, working with the Wellbeing for Life (WBFL) locality team, assigned to a specific Primary care network (PCN) area, will engage and work with a wide range of partners and local voluntary and community groups to develop joint health improvement activities ensuring that specific groups are reached by the service.

Responsibilities

  • Carry a caseload of clients aged 16 years and over to facilitate lifestyle and behavior change, particularly in the areas of weight management, smoking, mental wellbeing, and reducing alcohol intake.
  • Carry out promotional work to engage people in the service and offer one-to-one and group wellbeing advice and support to clients, including signposting them to relevant services within the community (NHS and non-NHS).
  • Act as an integrated member of a team, demonstrating a flexible approach, to working across the geographical location/patch to ensure the needs of the local community are met.

Requirements

To be considered for this role, applicants should have:

  • Experience in delivering health improvement programmes and interventions.
  • Experience in engaging with diverse communities and populations.
  • Experience in promoting health and wellbeing through various channels.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively with a variety of partners and stakeholders.
  • Knowledge of public health principles and practices.

Qualifications

The following qualifications are desirable but not essential:

  • BSc in Public Health, Psychology, Social Work, or a related field.
  • Chartered Institute of Environmental Health (CIEH) membership.

  • Strong organisational and time management skills.
  • Ability to work independently and as part of a team.
  • Effective problem-solving and decision-making abilities.
  • Excellent written and verbal communication skills.
  • Ability to manage multiple tasks and priorities.

Schedule
