About the Role
The Home and Community Based Services Licensed Practical Nurse provides direct nursing care in patients' homes, following physician prescriptions.
Responsibilities
- Deliver high-quality clinical services within scope of practice and state laws, adhering to the care plan and infection control protocols.
- Document patient visits thoroughly in electronic medical records and visit verification systems.
- Participate in care coordination, reporting patient needs and changes, and communicating effectively with patients, families, and healthcare team members.
Requirements
- Current, unrestricted LPN licensure in the state.
- CPR certification or ability to obtain within 90 days.
- Valid driver’s license, reliable transportation, or access to public transit.
- Ability to work independently in diverse home environments.

Pay
Compensation ranges from $22.90 to $34.35 per hour.
