Role Overview

The Home and Community Based Services Licensed Practical Nurse (LPN) provides direct nursing care to acutely and chronically ill patients in their homes, ensuring high-quality patient care in coordination with the healthcare team.

Responsibilities

• Deliver clinical services within legal and professional scope, following care plans and infection control standards.
• Accurately document patient visits and progress in electronic medical records and visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, physicians, and team members.

Qualifications & Skills

• Current, unrestricted LPN licensure in the practice state.
• CPR certification or ability to obtain within 90 days.
• Valid driver's license, vehicle insurance, and access to reliable transportation.
• Ability to work independently, manage multiple tasks, and communicate effectively.
