Weekend Hospice Nurse - Registered Nurse

About the role
The Weekend Registered Nurse (RN) will provide care and services for a group of hospice patients, visiting patients where they reside, which could include private homes, assisted living facilities, and hospitals. As an RN, you will complete hospice admissions and patient visits as needed. The Weekend RN is responsible for answering calls from patients, families, and customers, and making nursing visits for symptom management, deaths, and admissions as needed. This position must be passionate about hospice care and be a great advocate for both hospice services and Suncrest Hospice.
Responsibilities
- Provide care and services for hospice patients in various settings (private homes, assisted living facilities, hospitals)
- Complete hospice admissions and patient visits as needed
- Answer calls from patients, families, and customers
- Make nursing visits for symptom management, deaths, and admissions
- Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
- Work closely with patients on a 1-1 level while receiving support from a patient care team
- Work independently with activities including phone, email, and written communication
- Attend meetings with a variety of healthcare professionals
- Understand and adhere to hospice rules and regulations
- Manage visits, documentation, and meetings with strong time management skills
- Prioritize and triage multiple concerns to respond to urgent needs
- Learn and use an electronic medical records system
- Drive with reliable transportation across the service area
Requirements
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
- One year of experience under license
- Admissions and on-call experience preferred
- Valid driver’s license, reliable transportation, and auto insurance

Schedule
Saturday and Sunday, 8:00 AM – 8:00 PM (12-hour shifts)
