Weekend Hospice Nurse - LPN

Responsibilities

• Complete patient visits as needed, including in private residences, assisted living communities, and skilled nursing facilities.
• Answer calls from patients, families, and customers.
• Advocate for hospice services and represent Brighton Hospice.
• Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Provide 1‑to‑1 patient care while receiving support from the patient care team.
• Manage phone, email, and written communications, attend meetings with healthcare professionals, and adhere to hospice rules and regulations.
• Prioritize and triage multiple concerns, managing visits, documentation, and urgent needs.
• Learn and use the electronic medical records system.

Requirements

• Current Licensed Vocational/Practical Nurse (LVN/LPN) licensure in good standing in the state(s) of practice.
• Strong clinical and communication skills.
• Ability to work well within a team environment and collaborate with interdisciplinary staff.
• Capability to work independently, managing daily activities, communications, and documentation.
• Effective time‑management skills to schedule visits, attend required meetings, and prioritize urgent needs.
• Willingness to learn an electronic medical records system.
• Reliable transportation, valid driver’s license, and car insurance.

Pay

Pay scale $33‑$36 per hour plus bonus opportunities.

Schedule

Work Saturdays and Sundays in 8‑ or 12‑hour shifts, making needs‑based visits to patients in the community.
