Weekend Hospice LPN

About the Role

The Weekend Day Hospice Visit LPN provides compassionate nursing care to hospice patients during weekend shifts, focusing on holistic support for physical, emotional, and psychosocial needs. The role involves conducting assessments, administering medications, educating patients and families, and collaborating with interdisciplinary teams to ensure personalized care plans. The LPN must work independently, maintain clear communication, and document interventions accurately to promote patient comfort and quality of life in home or facility settings.

Requirements

• Valid LPN license in the U.S.
• At least one year of clinical nursing experience, preferably in hospice or palliative care.
• Basic Life Support (BLS) certification.
• Strong organizational, time management, and communication skills.
• Proficiency with electronic health records and basic computer skills.

Preferred Qualifications

• Experience in hospice or palliative nursing.
• Certification in Hospice and Palliative Nursing (CHPN) or similar.
• Training in symptom and pain management.
• Excellent interpersonal skills and knowledge of community resources.
