Weekend Hospice RN - RN & LPN Services

Oversees the development, implementation, and evaluation of the plan of care in collaboration with the interdisciplinary team to create the most positive hospice experience for the patient and family.

Responsibilities

• Verbalizes, demonstrates, and documents nursing care, change in condition, treatments, and procedures in compliance with Health First/Hospice Policy, Federal/State Regulations, and National Safety Patient Goals.
• Demonstrates effective medication administration and appropriate delegation and supervision of the CNA, including the care planning process and supervisory oversight.
• Implements consistent nursing practices across the 4 levels of Hospice care (Routine, Inpatient, Respite, and Crisis/Continuous Care).
• Provides patient, family, and/or caregiver educational material as directed by the plan of care within the interdisciplinary team.
• Provides service to patients and families with sensitivity and respect for their needs, expectations, age, cultural, spiritual, physical, and individual needs.
• Demonstrates fiscal responsibility with medication management, supplies, and other resources.
• Ensures accurate and complete documentation of patient transfer information between levels of care for billing department utilization.
• Prioritizes and manages diverse tasks or responsibilities while utilizing resources correctly.

Requirements

• Current Florida RN license and endorsement permit.
• Graduate of an Accredited School of Nursing.
• Minimum of two years’ experience in Oncology, Medical/Surgical, or Home Health.
• Current AHA BLS Healthcare Provider Completion Card (prior to start date and maintained).
• Successful completion of Level II background screening.
• Experience with Palliative Care/End of Life issues (helpful).
• Completes orientation checklist within three months of employment.
• Knowledge of regulatory guidelines (state and federal).
• Valid Florida driver’s license, insurance, and reliable transportation.

Physical Demands

• Ability to speak/understand written and verbal instructions in English.
• Visual acuity and hand-eye coordination to perform clinical tasks.
• Corrected vision and hearing to a normal range.
• Ability to lift, turn, pull, push, or ambulate patients/equipment.
• Ability to lift up to 40 lbs. unassisted.
• Ability to perform a complete physical assessment on patients.
• Ability to sit or stand for extended periods (up to four hours).
• Potential exposure to biohazardous waste and known or unknown diseases.
• Flexibility in work schedule to cover other areas of Hospice services (e.g., field program to cover call and home health to cover continuous care).

• Ability to concentrate on multiple priority activities.
• Critical thinking and prioritization skills.
• Emotional stability to cope with loss and end-of-life issues frequently.
