About the role
The Clinical Admissions Nurse at Hospice of the Western Reserve (now Reserve Care) is responsible for ensuring hospice programs are effectively represented to patients, families, and others interested in hospice services. This role involves performing assessments, providing information, and admissions services in patients' homes, nursing facilities, or healthcare facilities. The nurse collaborates with the Referral Department and other referral sources to promote the hospice philosophy of care.
Responsibilities
- Assesses patients referred to Hospice of the Western Reserve (now Reserve Care) and provides information, emotional support, and education to patients and families.
- Communicates with the Referral Department, referring physicians, and Hospice MDs to ensure referrals are appropriate and necessary certifications are obtained.
- Keeps the Referral Department informed about the progress of referrals and admissions.
- Applies nursing principles and techniques in the treatment of patients.
- Promotes teamwork by participating in interdisciplinary teams and sharing essential patient information.
- Completes and submits documentation within agency timelines that is complete, legible, and clearly reflects care given.
- Utilizes appropriate resources to maintain knowledge and skills for hospice care in various settings, including regulatory/reimbursement guidelines.
- Collaborates with other members of the interdisciplinary team to coordinate care efficiently and financially.
- Makes appropriate referrals to other disciplines and medical equipment/supply companies to meet patient care needs.
- Works with diverse social and economic situations without imposing personal values on patients or families, maintaining appropriate professional boundaries.
- Maintains licensure and demonstrates understanding of HIPAA privacy standards and related agency policies.
Requirements
- Graduate of an accredited nursing program: BSN and/or CHPN
- Experience with volunteer management and HR practices preferred
- Nurse Licensure by the state of Ohio
- Valid driver's license with an automobile that is insured in accordance with the state and/or agency requirements and is in good working order
- One year of nursing experience required: intake, home care, hospice, and/or oncology nursing experience required
Qualifications
- Minimum of one year of nursing experience required: intake, home care, hospice, and/or oncology nursing experience required
Skills
- Computer skills to access medical information and documents in electronic medical records as required
