On-Call Nurse
Join us in making the most of every day! As an On-Call Nurse with Hospice of the Western Reserve (now Reserve Care), you will have the autonomy to utilize your nursing skills to provide intermittent skilled nursing services to patients and families in typically urgent times. You will be invited into patients' homes and lives during uncertain times and have the honor of becoming a trusted face.
- Autonomy to utilize nursing skills
- Provide intermittent skilled nursing services
- Monitor and evaluate pain and symptoms
- Communicate with physicians as needed
- Provide emotional support to patients and families
- Refer patients and/or family members to other services
- Teach patients and families about care in a residence
- Make visits to patient's residence at time of death
- Maintain and promptly complete documentation
- Communicate and act as liaison with patients, family, and health professionals
- Participate in meetings as initiated by team leader
- Attend RC University events, trainings, or in-services as necessary
- Perform triage coverage as needed
Qualifications
- High School Diploma or equivalent
- Graduate of an accredited school of nursing, with a Bachelor's degree preferred
- Licensed RN in state of Ohio
- Experience preferred in pain and symptom management, oncology, or mental health
- Ability to communicate effectively, orally and in writing
- Ability to work independently and collaboratively with a team
- Demonstrated knowledge and skills to provide appropriate care to all age groups
- Ability to handle stressful and constantly changing conditions
- Ability to access patients and families in any residential and/or public setting in any weather condition
- Proficiency in Microsoft Office suite of products, particularly proficient in Word and Excel
- Maintain a valid driver's license, good driving record and auto accident insurance at a level acceptable to HWR and/or HWR’s insurer
Success Capabilities
- High School Diploma or equivalent
- Bachelor's degree in Nursing preferred
- Licensed RN in state of Ohio
- Experience in pain and symptom management, oncology, or mental health
- Effective communication skills
- Ability to work independently and collaboratively
- Knowledge and skills to provide appropriate care to all age groups
- Handling stressful and constantly changing conditions
- Accessing patients and families in any residential and/or public setting
- Proficiency in Microsoft Office suite of products
- Valid driver's license and auto insurance

Conditions of Employment

- Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
