Weekend Admissions Coordinator

Golden Heights Healthcare in Daly City, California is seeking a Weekend Admissions Coordinator to join our team. In this role, you’ll be a key point of contact for referrals, residents, families, hospitals, and healthcare partners. Your organization, communication, and professionalism will help ensure a smooth and welcoming admission experience. Key Responsibilities: Serve as the primary contact for all incoming referrals, calls, walk-ins, and inquiries.Coordinate admission documentation completion, insurance verification, and pre-admission authorizationCommunicate clearly and professionally with hospitals, case managers, residents, families, and internal departments.Ensure accurate data entry and timely updates in Point Click Care CRMFacilitate facility tours, referral follow-up, and participate conversion strategies.Maintain strong customer service standards at every touchpoint.Collaborate with nursing, therapy, and administrative teams to ensure smooth admissions.Ensure empty rooms are admit and tour ready on a daily basisAssist with marketing events, community outreach, and relationship development activities. Education/Requirements You’ll be a great fit if you are: Experienced – Prior SNF, rehab, or healthcare admission experience strongly preferred.Detail-driven – Organized, accurate, and confident managing multiple moving parts.Customer-focused – You communicate with empathy, clarity, and professionalism.Tech-savvy – Proficient in Microsoft Office, Point Click Care (PCC), Insurance Portals (NaviHealth, Availity, UHC, etc), Hospital Referral Portals (AIDA, CarePort, Wellsky, EPIC)Collaborative – A team player who works well with nursing, clinical, and leadership staff.Poised & dependable – You bring positivity, composure, and reliability every day.
