About the Role

We are seeking a skilled Registered Nurse Case Manager to join our dynamic team in the Redwood City-Sunnyvale area. This position involves making home visits to patients, conducting initial assessments, and collaborating with healthcare professionals to provide comprehensive care.

Responsibilities

• Utilize clinical skills to complete initial assessments and determine home care needs.
• Conduct Start of Care (SOC) admissions and follow up as needed.
• Make home visits to patients in the designated territory.
• Perform assigned duties, including medication reconciliation, wound care, treatments, and procedures.
• Maintain accurate documentation of observations, interventions, and evaluations.
• Collaborate with physicians and agency staff to deliver high-quality care.

Requirements

• Current and unrestricted CA RN license.
• Current BLS/CPR certification.
• Valid CA Driver’s License.
• Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired.
• High level of clinical knowledge in home health care.
• Experience with high acuity patients.
• Experience working with an interdisciplinary team.
• Case Management fundamentals.
• Self-directed, empathetic, critical thinker, and outstanding interpersonal skills.
• Leadership skills and organized.

Pay

Schedule
