About the Role
As a Registered Nurse Case Manager, you will utilize your outstanding clinical skills to complete initial assessments of patients to determine home care needs, conduct Start of Care (SOC) admissions, make home visits, perform assigned duties, monitor clients' conditions, collaborate with physicians and agency staff, and accurately document patient care management and services provided.
Responsibilities
- Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including: Conduct Start of Care (SOC) Admissions - OASIS
- Conduct follow ups, evaluation visits as needed in designated territory
- Make home visits to patients in designated territory
- Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
- Monitor clients’ conditions, reporting changes to Clinical team
- Collaborate with physicians and agency staff to deliver the best care possible
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
Requirements
- Current and unrestricted CA RN license
- Current BLS/CPR Certification
- Valid CA Driver's License
- Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired
- High level of clinical knowledge in home health care is highly preferred
- Experience with high acuity patients is a plus
- Experience working with an interdisciplinary team is highly desired
- Case Management fundamentals is highly desired

Qualifications
- High school diploma or equivalent
- Bachelor’s degree in Nursing preferred
- Current California Registered Nurse (RN) license
- Current Basic Life Support (BLS) certification
- Valid California driver’s license
- Experience with home health care
- Experience with high acuity patients
- Experience working with an interdisciplinary team
- Experience with case management
