Vocational Specialist - PACT Team I

About the Role

The Vocational Specialist provides clinical services focused on supporting vocational and educational goals, mental health counseling, and personal care services in accordance with the PACT (Program of Assertive Community Treatment) concept and philosophy. This role requires on-call and emergency response availability.

Responsibilities

• Contribute to resource development, supervision, training, and coordination of PACT consumers and their professional/vocational activities under the supervision of the Team Leader/Clinician.
• Conduct specialized assessments (vocational, educational, occupational, or rehabilitative) during intake or upon request by the consumer or PACT team members.
• Participate in intake screening, service planning, and discharge review processes, ensuring proper management and decision-making regarding services offered.
• Maintain accurate and current consumer records, including charts, progress notes, service/recovery plans, and other reporting mechanisms.
• Coordinate and advocate with community service providers for ongoing, emergency, and other forms of healthcare (e.g., medical, psychiatric, substance abuse).
• Conduct individual, group, and family education and counseling programs addressing vocational and pre-vocational needs.
• Represent the agency, PACT team, or consumer at community-based interdisciplinary service coordination meetings.
• Contribute to and participate in interdisciplinary in-service training and case conferences.
• Enhance consumer job readiness by networking with community businesses and maintaining a database of potential employment opportunities.
• Provide on-call and emergency response services.
• Participate in PACT Team and Agency Quality Assurance (QA) processes.
• Perform other duties as assigned by the Team Leader.
• Demonstrate commitment to PACT goals, philosophy, and the team’s response to consumer needs.
• Serve as a PACT team member, providing case management, medication monitoring, life-skill development, supportive counseling, crisis intervention, and specialized services.
• Establish and maintain treatment relationships with consumers, delivering face-to-face services in hospitals, communities, and other agencies or family settings.
• Participate in developing individualized treatment/recovery/rehabilitation/wellness plans, overseeing resource development and delivery of specialized services.
• Coordinate and provide PACT Team training in areas of clinical or management specialty.
• Provide direct and emergency back-up support to PACT team members.
• Provide 24/7 on-call services.
• Maintain accurate and complete documentation of services per established policy guidelines.
• Participate in supervision and program development, including field visits to consumer residences, work sites, or family settings.
• Engage in daily PACT Team planning, coordination meetings, and treatment planning conferences.
