Position Responsibilities
- Conduct virtual admissions, document patient history, allergies and baseline needs in EMR.
- Uses independent critical judgement in all aspects of patient care delivery.
- Provides support to bedside care team through direct observation via video monitoring and documents as appropriate.
- Rounds virtually for assigned patients and documents patient response per established guidelines in the EMR.
- Succeeds in ACLS training and maintains current status within 90 days of hire.
- Establishes an effective communication plan with bedside nurses/PCTs to address changes in patient condition.
- Demonstrates understanding of patient multi-system complications, evidenced by identification and reporting of abnormal findings to appropriate physician in a timely manner and implementation of nursing interventions.
- Collaborates with all members of the health care team including physicians, when planning patient care and documents accordingly.
- Virtual Support
- Performs virtual admissions, including history taking, medication reconciliation, preferred pharmacy.
- Obtains and documents a complete nursing admission assessment, including physiological and psychological factors to develop an interdisciplinary plan of care within eight hours of admission.
- Delivers personalized patient education at admission and discharge using telehealth platform.
- Conducts virtual rounding to assess patient needs and escalates concerns to bedside nurse.
- Affirms patient education needs on admission and daily.
- Affirms psychosocial needs specific to patient's age, growth, and development.
- Technology Utilization
- Uses EPIC EMR to document virtual interactions.
- Leverages Virtual Platform technology and real time communication tools.
- Communication and Collaboration
- Acts as a liaison between patients and care team.
- Communicates with bedside RN, providers, and case managers as indicated.
- Promotes patient safety and education through virtual support.
- Supports bedside staff by co-signing admission/discharge documentation as required.
- Performs medication reconciliation, safety checks, and patient teaching through Virtual Platform.
- Provides individualized instructions each shift to patient/significant other regarding the plan of care.
- Demonstrates safe and correct use of unit specific equipment (Virtual platform).
- Supports bedside staff by co-signing admission/discharge documentation as required.
- Performs discharge education and instructions via secure video, ensuring patient/family comprehension and addressing questions.
- Evaluation
- Documents/reports patient responses or outcomes resulting from: nursing interventions, treatments/procedures, interpersonal interactions, and patient/family education.
- Professional and Staff Development
- Obtains 100% of mandatory hospital and unit specific in-services annually, prior to evaluation date.
- Plans, presents, and documents unit inservice as required by level.
- Documents competency in clinical skills annually.
- Attends staff meetings as required.
- Uses established lines of communication for resolution of problems and interpersonal conflict.
Qualifications
- Current FL Nursing License, Current Basic Life Support and Current Advanced Cardiac Life Support required.
- Graduate from an Accredited School of Nursing.
- Proficient with Windows-based computer skills and operations.
- Proficient with electronic medical records.
