About the role

The Case Worker plays a crucial role in providing support to clients facing a wide range of challenges. They conduct comprehensive interviews with individuals dealing with personal and family adjustments, potential foster care situations, financial struggles, employment issues, and various other concerns related to food, clothing, housing, and physical or mental impairments. Through these interviews, the Case Worker gathers important information about the clients' medical, psychological, and social factors, enabling them to assess the nature and severity of the problems. They then offer counseling services on an individual or group basis, assisting clients in developing plans to address their needs and empowering them to tap into their inner strengths and external resources for improved social functioning.

Responsibilities

  • Conduct comprehensive interviews to gather information about clients' medical, psychological, and social factors
  • Assess the nature and severity of clients' problems and challenges
  • Offer counseling services on an individual or group basis
  • Assist clients in developing plans to address their needs
  • Empower clients to tap into their inner strengths and external resources for improved social functioning
  • Regularly meet with clients as required by funding sources
  • Monitor client progress and provide guidance to modify attitudes and behaviors for positive change
  • Connect clients with community resources and organizations to establish a network of support
  • Complete necessary reporting forms and maintain accurate client case files
  • Ensure effective communication with supervisors and funding sources
  • Remain vigilant about client safety and promptly notify relevant parties of any potential safety issues

Qualifications

  • Associate Degree in Human Service or equivalent (combination of education and experience can be considered)
  • At least three years of experience working with vulnerable individuals
  • Candidates with lived experience are preferred and encouraged to apply
  • Valid N.Y.S. Driver's License
  • Registered and Insured Vehicle
  • N.Y.S. Child Abuse Registry Clearance

Working conditions

  • Mobility sufficient to drive to and participate in meetings at client's homes and/or community facilities that may not meet Federal accessibility standards for disabled individuals
  • Manual dexterity sufficient to work with children/adults in a challenging and active environment

Pay

$22.00 per hour

Schedule

Full Time at 35 hours a week
