Veteran Program Intake & Retention Specialist
Job Summary
St. Patrick Center, a ministry of Catholic Charities of the Archdiocese of St. Louis, seeks an Intake and Retention Specialist to coordinate HERO Program intakes and admissions, collect required documentation, and provide follow-up case management for up to one year for clients transitioning to permanent housing. Core working hours are Monday through Friday 8am to 4:30pm.
Overview and Responsibilities
- Visit client monthly to track ISS goals for one year after transition to permanent housing.
- Screens for suicidal and homicidal risks.
- Make phone calls to client, landlord, natural supports, employer, etc. if unable to meet in person.
- Document each visit/attempt in CaseWorthy.
- Provide support and resources as needed/available to aid clients in achieving ISS and treatment goals.
- Contribute to the ideas and processes of the program case management team.
- Aid in housing placement based on the desires of clients.
- Aid team members in addressing the needs of the clients by providing vouchers, hygiene kits, job referrals, or any other necessary referrals.
- Coach clients to make solid, well-informed decisions.
- Develop or contact family and natural supports which exist in the community.
- Collaborate with the wrap-around services developed to ensure the proper releases are signed by clients.
- Assess the clients’ strengths, desires, and immediate/long-term needs.
- Receive input from case management team regarding assessment of employment, housing, mentoring, substance abuse, and other immediate/long-term needs.
- Complete daily and monthly departmental paperwork.
- Update daily clients’ electronic files in CaseWorthy and include any types of contact with client, services, or any internal or external referrals made.
- Refer client to additional resources to address other service needs (i.e., legal assistance, VA benefits, community substance abuse counseling, etc.).
- Research additional benefits available to enhance service being provided.
- Engage the client, the family and natural supports within the community setting when feasible and desired by the client.
- Meet with referrals weekly to determine eligibility. If deemed ineligible, refer to appropriate program.
- Complete all initial paperwork by providing informed consent of program guidelines and procedures.
- Complete all property management paperwork and coordinate with VA on move in date.
- Document intakes and referrals into CaseWorthy.
- Administer Waived Testing - drug urinalysis testing.
Knowledge and Experience Requirements
- Bachelor’s degree from an accredited university or college required.
- BSW preferred, or equivalent experience with proof of ability to perform effective case management.
- Driver License is required (MO residents require Class E), personal vehicle, and auto insurance.
- Trauma informed care trained.
- Knowledge of housing first and harm reduction.
- Knowledge of housing and other supportive service resources.

Pre-Employment Screening
All candidates receiving an employment offer must submit a pre-employment screening. The screenings will include criminal background check and other background checks (as needed), Family Care Safety Registration, drug screen, an employment and education or licensure/certification verification, and Covid-19 and Influenza vaccination.
