VEN Centers Outreach Specialist (Mohave County)

VEN Centers, Powered by Arizona Liver Health (ALH), is a medical practice leading the charge in combatting the elimination of viral hepatitis in Arizona. With extensive expertise in hepatology and a deep-rooted passion for the field, we adopt innovative strategies to bring Hepatitis C virus (HCV) treatment and cure directly to patients, removing obstacles and establishing a new benchmark in healthcare.

Status: Full-time, Exempt
Salary: $50,000 to $65,000 per year, depending on experience and qualifications
Location: This role is remote with 50-75% travel to pop-up sites; not office-based.

Responsibilities

• Research and coordinate with local health departments, community groups, medical facilities, and treatment centers for community outreach and collaboration on services offered for Viral Hepatitis and STIs.
• Conduct outreach to Viral Hepatitis high-risk communities.
• Provide individualized education and risk reduction plans to people at risk of drug-related harm, overdose, and other health disparities.
• Complete patient testing for Viral Hepatitis and other STIs.
• Conduct psychosocial assessments to identify non-medical needs (e.g., social, emotional, or economic factors) that may interfere with treatment.
• Provide support to HCV/STI-positive persons interested in starting treatment and those already in treatment; identify and re-engage patients who have fallen out of care.
• Utilize electronic medical records and reporting systems for surveillance and tracking of patient results and treatment adherence.
• Maintain appropriate documentation regarding psychosocial assessments and treatment rendered following utilization review standards issued by healthcare regulatory agencies.
• Maintain regular contact with patients and document interactions, including phone calls and conferences.
• Maintain accurate record-keeping, client tracking, data collection, and monthly reports.
• Adhere to HIPAA statutes and client confidentiality policies.
• Remain current with HIV/HCV/STI trends, developments, and state-of-the-art prevention efforts.
• Identify patients who test positive for Hepatitis in collaboration with community resources.
• Arrange and provide group education for Viral Hepatitis/STIs in collaboration with community partners.
• Perform additional duties as assigned.

Requirements

• Bachelor's degree in a relevant field such as public health, community health, nursing, social work, or a related discipline.
• Significant experience and training in infectious disease, substance treatment, human services, or a related field preferred.
• Proficient in Microsoft applications (Outlook, Word, Excel, PowerPoint, OneDrive) and the practice’s EMR system (eCW).
• Excellent verbal and written communication skills.
• Demonstrated ability to work independently and efficiently in a remote work environment, managing time and priorities effectively.
• Ability to provide patient care and outreach with minimal supervision.
• Proficiency in using remote communication tools such as video conferencing, email, instant messaging, and project management platforms.
• Demonstrates awareness of quality-of-care metrics and regularly meets them.
• Strong interpersonal skills and ability to maintain good working relationships with peers, coworkers, and patients.
• Must be open to working with people from diverse backgrounds, cultures, and lifestyles in a non-judgmental manner.
• Flexibility to adapt to changing work environments and schedules, including occasional evening or weekend activities.
• Knowledge of public health principles, including prevention and transmission of HIV, Hepatitis C (HCV), other Sexually Transmitted Infections (STIs), and harm reduction for People Who Inject Drugs (PWID).
• Phlebotomy experience preferred.
• Bilingual (Spanish-speaking) highly preferred.
• Ability to problem-solve systems and processes.
• Must have valid documentation and authorization to work in the U.S.; visa sponsorship is not available.
