About the role

Responsibilities
- Safe and timely transportation of Senior Community Care (SCC) participants
- Transports participants to and from medical/specialist appointments, and activity outings, as requested by the Interdisciplinary Team via the plan of care
- Transports meals, durable medical equipment, pharmacy, and any other items as deemed necessary for participant
- Provides care in a manner which ensures participant satisfaction during transport
- Fulfills assigned schedule, adjusts to meet unanticipated changes, while considering the needs of the participant and requirements of the PACE Center
- Communicates any changes to dispatch
Requirements
- High school diploma or equivalent required
- Valid Driver’s License in state of Colorado
- One year experience in working with the frail and elderly population
- Able to read and understand maps and routing information
- Knowledge of safety requirements pertaining to the transportation industry

What You Bring

Pay & Schedule
$19.50 - $23.50 per hour, based on previous work experience. Schedules: Monday - Friday 7:30am - 4:30pm with occasional weekends.
