About the role

Value-based Primary Care Physician role to manage chronic conditions of high-risk geriatric patients Monday-Friday (no weekends or evenings). Spend 30-45 minutes with your patients to manage all chronic conditions alongside a supportive team of providers and office operations staff.

Responsibilities

• Deliver primary health care and patient care management in a residential setting.

Requirements

• Active/unrestricted medical license and DEA.
• Board Certified or Board Eligible.
• EMR experience.
• Must maintain a valid driver’s license and good driving record.

Qualifications

• Experience in Geriatrics or complex patients preferred.

Individual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related considerations.
