Position Summary

The LPN/RN Unit Manager is responsible for the clinical oversight, staffing coordination, and day-to-day operational management of an assigned nursing unit within a skilled nursing facility (SNF). This role serves as a critical link between frontline nursing staff and facility leadership, ensuring that residents receive high-quality, person-centered care in accordance with federal and state regulations, facility policies, and clinical best practices. The Unit Manager is expected to demonstrate strong leadership, sound clinical judgment, and a proactive approach to quality improvement and regulatory compliance.

Essential Functions & Responsibilities

  • Clinical Oversight

    • Supervise and direct nursing staff (CNAs, LPNs, RNs) on the assigned unit across all shifts
    • Conduct and document comprehensive nursing assessments for newly admitted and current residents
    • Develop, review, and update individualized care plans in collaboration with the interdisciplinary team (IDT)
    • Monitor residents for changes in condition; initiate appropriate interventions and physician notification in a timely manner
    • Oversee wound care, medication management, and treatment protocols for assigned residents
    • Participate in IDT meetings, care conferences, and family meetings as required
    • Collaborate with therapy, social services, dietary, and activities to ensure holistic resident care
  • Staffing & Scheduling

    • Assist the DON with unit-level scheduling to maintain appropriate staffing ratios per shift
    • Address call-outs and staffing gaps in real time; coordinate with staffing coordinators or agency vendors as directed
    • Monitor and enforce compliance with agency utilization targets and overtime thresholds
    • Evaluate daily staffing levels against census and acuity; escalate concerns to leadership as needed
  • Regulatory Compliance & Documentation

    • Ensure all clinical documentation is accurate, complete, and timely in accordance with CMS requirements and facility policy
    • Support MDS coordination by providing timely clinical data for accurate RAI/MDS completion
    • Participate in preparation for and response to state surveys (AHCA/ODHS inspections); assist with plan of correction development
    • Monitor F-tag risk areas on the unit (F600–F699 range, resident rights; F700s, quality of care); implement corrective actions proactively
    • Maintain compliance with HIPAA, 42 CFR Part 483, and all applicable Georgia Department of Community Health (DCH) regulations
  • Quality & Performance Improvement

    • Track and trend unit-level quality indicators including falls, pressure injuries, infections, and rehospitalizations
    • Conduct regular chart audits and nursing documentation reviews
    • Contribute to reducing avoidable rehospitalization rates through proactive clinical monitoring and communication with attending physicians
    • Implement and monitor interventions to address identified deficiencies
  • Staff Development & Leadership

    • Provide coaching, mentoring, and performance feedback to nursing staff on the unit
    • Identify training needs and collaborate with the Staff Development Coordinator to address clinical competency gaps
    • Participate in performance evaluations for unit staff in coordination with the DON
    • Foster a culture of accountability, professionalism, and respect among the care team

  • Communication & Administrative Duties

    • Conduct or participate in shift-to-shift huddles to ensure continuity of care and communication
    • Maintain open lines of communication with residents, families, and responsible parties regarding care concerns
    • Submit required unit reports (staffing summaries, incident logs, supply requests) to leadership on schedule
    • Aid in onboarding of new nursing hires assigned to the unit

Qualifications

  • Education & Licensure

    • Active, unrestricted LPN or RN license issued by the Georgia Board of Nursing — required
    • Associate's Degree in Nursing (ADN) or higher preferred for RN applicants
    • CPR/BLS certification required for LPN applicants (or willingness to obtain within 90 days of hire)
  • Experience

    • Minimum 2 years of clinical nursing experience in a long-term care, skilled nursing, or post-acute rehabilitation setting — required
    • Minimum 1 year of charge nurse, supervisory, or unit management experience — preferred
    • Experience with Point Click Care (PCC) or similar SNF EHR platform strongly preferred
    • Familiarity with MDS/RAI process and CMS SNF survey protocols preferred
  • Knowledge, Skills & Abilities

    • Working knowledge of 42 CFR Part 483 Conditions of Participation and Georgia SNF regulations
    • Strong clinical assessment and critical thinking skills
    • Demonstrated ability to lead, motivate, and hold nursing staff accountable
    • Proficiency in electronic health records and Microsoft Office applications
    • Excellent oral and written communication skills
    • Ability to manage competing priorities and work effectively under pressure
    • Commitment to resident-centered care and maintaining a dignified living environment
