Unit Manager-LPN

About the role

The Charge Nurse supervises and supports a nursing staff, onboards patient care, and treats patients as needed. They are responsible for maintaining a high level of patient care, evaluating other nurses, and acting as an educational resource for nurses. This is a high-volume, fast-paced position requiring someone who can work well under pressure and is comfortable multitasking.

Responsibilities

• Direct the admission, discharge, and general care of patients
• Provide guidance on administering care to new patients or those with special needs and answer questions
• Create and implement individualized care plans in collaboration with other nurses and patients
• Develop and implement training courses and organize needs assessments to educate and train new nurses and staff
• Inform nursing personnel of new admissions, their expected time of arrival, room assignment, and other pertinent information
• Ensure rooms are ready for new admissions; work closely with the C.N.A. team to make every new admission feel comfortable and welcomed
• Communicate effectively with physicians, care team, residents, and family members
• Requisition and arrange for diagnostic and therapeutic services as ordered by the physician and in accordance with established procedures
• Consult with the resident’s physician and care team in providing the resident’s care, treatment, and rehabilitation
• Notify proper staff, personnel, and agencies when a resident is involved in an accident or incident
• Carry out restorative and rehabilitative programs, including self-help and care
• Answer call lights and assist residents in a timely manner
• Administer services within the applicable scope of nursing practice
• Ensure residents follow dietary allowances; report problem areas to Nursing Leadership and Dietary Supervisor
• Admit, transfer, and discharge residents as appropriate
• Assist in arranging transportation for residents as needed
• Uphold and respect the rights of residents in a skilled nursing facility
• Maintain Relias training and in-service requirements
• Follow drug administration protocol, including disposal of drugs and narcotics as required
• Perform charting duties as required, including timely, accurate, and thorough documentation of care provided and resident responses
• Assist in planning the nursing services portion of the resident’s discharge plan
• Chart nurses’ notes in a professional and appropriate manner
• Review care plans daily to verify appropriate care is being rendered; communicate with the care team when necessary

Requirements

• Must be an experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN)
• Demonstrated leadership, management, and communication skills
