Duties

  • Observe and monitor residents’ health and assess their needs to provide quality care.
  • Work with physicians to create individual care plans and assist them during required operations or procedures.
  • Maintain complete healthcare records and reports including PCC being utilized according to best practices.
  • Serve as a clinical operational liaison between units and the DON, medical director, and other nursing staff.
  • Supervise, manage, and assist in training LPN’s and GNA’s on staff.
  • Perform routine procedures as needed, including administering medication and injections, and provide emergency medical care when necessary.
  • Maintain a high standard of hygiene and cleanliness within the facility.
  • Collaborate with DON in maintaining adequate staffing coverage.
  • Effectively utilize existing manpower to limit the amount of overtime/agency needed to support staffing.
  • Assess work performance of nursing staff while on duty ensuring timeliness in services to our patients/residents.
  • Maintain security of the facility, creating an environment that is respectful, team-oriented, and responsive to the needs of our staff, patients, and their families.
  • Monitor nursing care to reduce hospitalizations and ensure positive outcomes.
  • Encourage shift to shift communication between incoming and outgoing nursing staff.
  • Review prospective Admissions in relation to existing nursing capabilities and ensure readiness to treat.
  • Ensure that patient's attending physician and family or responsible party are promptly notified of any significant change in the patient health condition.
  • Investigate patient/family complaints and take appropriate actions to bring to resolution.

Requirements

  • Current state RN nursing licensure
  • BLS/CPR certification
  • In-depth knowledge of nursing care methods and health and safety guidelines
  • Responsible and compassionate with outstanding organizational and communication skills
  • Experience supervising GNAs and LPNs in a SNF nursing home setting (min. 1 year)
