Duties
- Observe and monitor residents’ health and assess their needs to provide quality care.
- Work with physicians to create individual care plans and assist them during required operations or procedures.
- Maintain complete healthcare records and reports including PCC being utilized according to best practices.
- Serve as a clinical operational liaison between units and the DON, medical director, and other nursing staff.
- Supervise, manage, and assist in training LPN’s and GNA’s on staff.
- Perform routine procedures as needed, including administering medication and injections, and provide emergency medical care when necessary.
- Maintain a high standard of hygiene and cleanliness within the facility.
- Collaborate with DON in maintaining adequate staffing coverage.
- Effectively utilize existing manpower to limit the amount of overtime/agency needed to support staffing.
- Assess work performance of nursing staff while on duty ensuring timeliness in services to our patients/residents.
- Maintain security of the facility, creating an environment that is respectful, team-oriented, and responsive to the needs of our staff, patients, and their families.
- Monitor nursing care to reduce hospitalizations and ensure positive outcomes.
- Encourage shift to shift communication between incoming and outgoing nursing staff.
- Review prospective Admissions in relation to existing nursing capabilities and ensure readiness to treat.
- Ensure that patient's attending physician and family or responsible party are promptly notified of any significant change in the patient health condition.
- Investigate patient/family complaints and take appropriate actions to bring to resolution.
Requirements
- Current state RN nursing licensure
- BLS/CPR certification
- In-depth knowledge of nursing care methods and health and safety guidelines
- Responsible and compassionate with outstanding organizational and communication skills
- Experience supervising GNAs and LPNs in a SNF nursing home setting (min. 1 year)
