About the role
Under general supervision of the TB Unit Manager, provides social work services for persons receiving care for TB. Works with the TB Nurse Case Manager to promote completion of recommended treatment, including medication delivery for TB infection and/or disease, and assists patients with initiation of Video Directly Observed Therapy (VDOT).
Responsibilities
- Provides counseling, emotional/mental health support, and linkage to care for clients dealing with chronic illness, mental health disorders, and substance use concerns.
- Conducts psychosocial assessments, treatment, and follow-up services for a diverse population, addressing substance abuse and mental health needs.
- Offers short-term therapy to help individuals and families adjust to health-related problems, assists in developing long-range health plans, and makes referrals to community resources.
- Develops discharge plans when social work services end, with goals and treatment plans set in collaboration with the individual/family.
- Ensures documentation meets policy standards and maintains patient confidentiality.
- Provides patient education, initiates referrals, and counsels individuals, caregivers, and families in clinic, household, and community settings.
- Assists with contact investigations as needed.
- Facilitates patient access to community resources and assists in discovering resources for persons with active or suspected Tuberculosis (TB) or Latent TB Infection (LTBI).
- Participates in the development of social work program policies as indicated.
- As Unlicensed Assistive Personnel (UAP):
- Provides medication delivery for treatment of TB infection and/or disease.
- Assists patients with initiation of Video Directly Observed Therapy (VDOT).
- Notifies Nurse Case Manager immediately of any missed appointments.
- Utilizes incentives and enablers to assist with treatment adherence.
- Provides groceries and transportation with TB Unit Manager approval.
- Administrative Duties:
- Participates in Continuous Quality Improvement (CQI) activities such as record reviews, program evaluations, and team meetings.
- Maintains records and reports in accordance with agency standards, including documentation in the Electronic Health Record and required agency reports.
- Conducts Social Work peer audits as designated.
- Emergency Response:
- Assists with agency shelter operations and client emergency response, including First Aid, CPR, and AED.
- Available to assist during public health emergencies and outbreak response efforts.
- Performs other duties as assigned.
Requirements
- Bachelor’s degree in social work from a program accredited by a nationally recognized accrediting body or equivalent.
- Licensure as a Licensed Baccalaureate Social Worker (LBSW) or Licensed Masters Social Worker (LMSW) by the South Carolina Board of Social Work Examiners.
- Must be a Licensed Independent Social Worker (LISW) by the SC Board of Social Work.
- Valid driver’s license; may be required to use personal vehicle.
- Ability to make home visits, including navigating stairs and encountering domestic animals.
- Significant travel and/or overnight stays within the state may be required.
- Flexible work hours may be required.
- Must pass a fit test and use appropriate respiratory protective equipment.
- Will be in contact with infectious or potentially infectious tuberculosis (TB) cases/suspects.
- Must obtain Basic Life Support (BLS)/CPR/First Aid certification within thirty (30) days of hire or provide documentation of prior completion.
- Must participate in disaster preparedness during hazardous weather or emergencies; subject to duty and/or call on a 24-hour basis.
