About the role
The WelbeHealth Traveling Occupational Therapist coordinates and provides restorative and rehabilitative occupational therapy services to participants at day centers and participant homes. This position works closely with the PACE Physician, rehabilitation staff, and other interdisciplinary team members to enhance participant independence and safety.
Responsibilities
- Conduct occupational therapy assessments in facilities and participant homes to define care needs, preferences, and goals
- Participate in interdisciplinary team meetings to develop participant care plans
- Deliver and document occupational therapy interventions according to physician orders or care plan agreements
- Train and instruct Therapy Assistants on implementing Functional Maintenance Programs (FMP)
- Train and instruct staff, caregivers, participants, and families on the use and care of therapeutic equipment
Requirements
- Required to travel 100% of the time for up to 2 months at a time, based on business needs
- Minimum of five (5) years of progressive occupational therapy experience
- Minimum of two (2) years of occupational therapy experience in a clinical setting with a frail or elderly population
- Bachelor’s degree in occupational therapy required; master’s degree preferred
- Valid California Occupational Therapy license, or proof of license eligibility
