Travel Social Worker Medical Social Worker Job
Overview
This 5-week travel assignment starts 08/17/2026, pays $1,973–$2,094 per week, and requires a 40-hour workweek on days in an inpatient care setting.
Responsibilities
- Provide counseling, support and assistance to patients and families to address needs incidental to hospitalization
- Conduct psychosocial assessments and identify barriers to discharge; coordinate follow-up as needed
- Develop and implement transition plans; facilitate post-discharge planning with outside agencies, nursing staff and relatives
- Offer direction for routine non-medical problems affecting patients and families and assist with resource navigation
- Document interactions, maintain accurate records, and communicate progress with the multidisciplinary team
Requirements
- Active California social work license or eligibility to obtain licensure
- Master of Social Work (MSW) degree from an accredited program
- Experience or strong familiarity with medical/inpatient social work and discharge planning
- Demonstrated counseling, communication and collaboration skills within a multidisciplinary team
- Ability to work on-site in an inpatient care setting and handle sensitive family interactions
