Responsibilities

• Conduct psychosocial assessments for patients and families to guide care planning
• Develop and coordinate discharge plans, ensuring appropriate post-acute services and community supports
• Identify psychosocial barriers to treatment, safety, and adherence; connect patients with resources
• Collaborate with the interdisciplinary team, participate in care conferences, and advocate for patient and family needs
• Provide crisis intervention when needed and document assessments and plans in the electronic health record

Requirements

• Active Georgia LCSW license
• Minimum 2 years of experience in a Medical Social Work specialty
• Acute care hospital experience or equivalent in a high-demand inpatient setting
• Ability to work night shifts within a fast-paced care environment
• Strong communication, collaboration, and documentation skills
