TRAVEL - Social Worker

About the Role

This is a travel position for a Social Worker at HCA HealthONE Swedish, located at 501 E Hampden Ave, Englewood, CO 80113. The role primarily involves case management with some weekend work required. Shifts are 8:00 AM to 4:30 PM, either Friday-Monday or Monday-Friday.

Unit Information

• Number of Beds: Units are mainly 20 beds each.
• Staffing: Team of Case Managers/Social Workers to manage workload.
• Patient Ratios: Social Worker 1:40, RN 1:20.
• Equipment: Mobile and computer.
• EMR Systems: Meditech, Midas, CarePort.

Responsibilities

• Discharge planning and assessments.
• SBIRT (Screening, Brief Intervention, and Referral to Treatment) and substance use assessments.

Requirements

• Minimum 1-year experience as a Social Worker, preferably in an acute care setting.
• Active Colorado licensure.

Preferred Qualifications

• Discharge planning experience.
• Emergency Department (ED) experience.
• Palliative care knowledge and Goals of Care (GOC) conversations.
• Behavioral health experience, particularly with substance use assessments.
