Social Worker

About the role

As our Social Worker, you will collaborate with the Registered Nurse Manager for timely discharge planning, assessing psychosocial impacts of illness, and providing direct patient services utilizing community resources. You will perform psychosocial assessments, discharge planning and coordination, community referrals, crisis intervention, and supportive counseling.

You'll play an integral role in the Multi-disciplinary Team, managing patient services, determining abuse or neglect referrals, coordinating care for psychiatric patients, ensuring safe discharges, and educating doctoral residents.

Responsibilities

• Assess, develop, implement, and monitor a comprehensive plan of care through an interdisciplinary team process in conjunction with the patient and family in internal and external settings.
• Comprehensively assess patient’s goals as well as their biophysical, psychosocial, environmental, discharge planning needs, and financial status.
• Work collaboratively with care partners to develop an appropriate discharge plan and ensure indicated referrals are completed in a timely manner.
• Procure services and serve as an advocate on behalf of patients and families for scarce resources.
• Act as a liaison to post-hospital care providers and community health resources.
• Educate patients, families, and medical staff regarding available resources and guidelines associated with accessing resources; provide applications as needed.

Requirements

• Certified Social Worker (CSW) in Nebraska upon hire.
• Minimum of a bachelor's degree in Social Work (BSW), with the intention of entering into a master's degree program in Social Work (MSW).
• Basic Life Support (BLS) from the American Heart Association upon hire.
• Ideally, experience in a medical setting, with Medicare/Medicaid populations, and in patient education.
• Strong assessment, planning, and advocacy skills within a team-based setting.

Where You'll Work

Established by the Sisters of the St. Francis in 1924, CHI Health Good Samaritan is a 268-bed regional referral center in Kearney, Neb. The facility includes an advanced level trauma center with AirCare emergency helicopter transport and an ambulance program providing long-distance and local 911 service. Services also include cardiac care (including open-heart surgery), a level II neonatal intensive care unit, a nationally accredited cancer center, comprehensive neurosurgery, an advanced orthopedic program, and inpatient and outpatient mental health services at the free-standing Richard Young Behavioral Health facility.

Pay
