About the role

Home Health LMSW responsible for OASIS assessments, care coordination, patient advocacy, and daily home visits within a 60-mile radius. Reliable personal vehicle required.

Responsibilities

• Conduct OASIS assessments in patients’ homes
• Coordinate care plans and advocate for patient needs
• Perform daily home visits following efficient routing (up to ~60 miles / 60–90 minutes total drive time)

Requirements

• Active or pending Licensed Medical Social Worker (LMSW) license
• Minimum 1 year of skilled Home Health experience
• Basic Life Support (BLS/BCLS) certification
• Completion of primary COVID-19 vaccination series plus one booster
• Reliable personal vehicle for daily travel
