Responsibilities
- Provide clinical social work services to patients in an outpatient clinic setting.
- Collaborate with healthcare providers to develop and implement treatment plans.
- Assess patient needs and provide counseling and support.
- Utilize Epic EMR for documentation and record keeping.
- Teach hospital skills to patients and families as needed.
Requirements
- Active California Licensed Clinical Social Worker (LCSW) license.
- Minimum of 2 years of clinical social work experience.
- Recent pediatric social work experience.
- Outpatient clinic experience.
- Case management experience.
- Experience using Epic EMR.
- Teaching hospital experience required.
