Responsibilities

• Manage geriatric patient caseload averaging 5‑7 patients per day.
• Provide timely documentation and communication as required.
• Maintain flexibility to work within up to a 60‑mile radius from the hub/office.
• Adhere to hospice‑specific case management and clinical social work duties.

Requirements

• Minimum 2 years of recent experience as a Licensed Clinical Social Worker.
• Current BLS certification.
• Valid social work license in hand (state of Washington).
• Ability to work locally; locals accepted at travel rate.
• Scrubs of any color required (no jeans).
• Cell phone use not permitted; iPad provided for work.
