Travel Social Work - Licensed Clinical Social Worker - $2,723 per week
Responsibilities
- Conduct comprehensive psychosocial assessments for patients and families.
- Develop and coordinate individualized discharge plans.
- Provide crisis intervention and emotional support.
- Collaborate with physicians, nurses, case managers, and interdisciplinary teams.
- Connect patients with appropriate community resources and support services.
- Maintain accurate and timely documentation in the electronic medical record.
- Float to various units and provide coverage as needed.
- Ensure compliance with hospital policies and professional standards.
Requirements
- Licensed Clinical Social Worker (LCSW) – Required.
- Minimum 4 years of recent LCSW experience – Required.
- BLS Certification – Required.
- Ability to work across multiple hospital units and adapt to changing patient care needs.
- Strong communication, assessment, and care coordination skills.
