Travel Skilled Nursing Facility (SNF) Physical Therapist - $2,846 per week

About the role

Triage Staffing is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week assignment starting August 3, 2026, working 40 hours per week, 8-hour shifts, Monday through Friday, with the possibility of floating between Sheboygan and Fon du Lac.

Responsibilities

• Provide physical therapy services to patients in SNFs
• Develop and implement treatment plans based on patient needs
• Assess patient progress and adjust treatment plans accordingly
• Collaborate with interdisciplinary teams to ensure patient care
• Document patient care activities and progress in medical records

Requirements

• Current licensure as a Physical Therapist in Wisconsin
• Experience in SNF settings preferred
• Knowledge of rehabilitation techniques and protocols
• Excellent communication and interpersonal skills

Qualifications

• Master’s degree in Physical Therapy from an accredited program
• Passing scores on the National Physical Therapy Examination (NPTE)
