About the role

We are seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week assignment in Saint Simons Island, Georgia. This role offers the opportunity to make a significant impact in a SNF setting.

Responsibilities

• Assess patients' physical abilities and develop personalized treatment plans
• Provide therapy to improve mobility, strength, and functional independence
• Collaborate with healthcare teams to ensure comprehensive patient care
• Educate patients and families on exercise programs and injury prevention
• Monitor and document patient progress and adjust treatment plans as needed
• Implement interventions in various settings
• Participate in discharge planning and coordinate follow-up care for continued rehabilitation

Requirements

• Doctorate in Physical Therapy
• GA Physical Therapist PT license
• Current BLS/CPR Certification

Pay

$2,604.83 - $2,685.39 per week

Schedule

• 13-week assignment
• 40 hours per week
• 8-hour day shifts
• On-site at a Skilled Nursing Facility (SNF)
