About the role
Ardor Health Solutions is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week assignment in West Caldwell, New Jersey.
Responsibilities
- Provide physical therapy services to patients in a skilled nursing facility setting.
- Develop and implement individualized treatment plans based on patient needs.
- Assess patient progress and adjust treatment plans accordingly.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Document patient care activities and progress in accordance with regulatory requirements.
Requirements
- Current state licensure as a Physical Therapist.
- At least one year of recent clinical experience in a skilled nursing facility setting.
- Bachelor’s degree in Physical Therapy from an accredited program.
- Passing scores on the National Physical Therapy Examination (NPTE).
Qualifications
- Excellent communication skills.
- Ability to work independently and as part of a team.
- Strong organizational and time management skills.
- Valid driver’s license and reliable transportation.
Skills
- Proficient in using electronic health records (EHR) systems.
- Knowledge of Medicare and Medicaid regulations.
- Experience with fall prevention and patient safety protocols.
