Travel Skilled Nursing Facility (SNF) Physical Therapist - $2,406 per week

Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments.
• Develop individualized treatment plans based on patient goals and physician orders.
• Treat orthopedic, neurological, post-surgical, and medically complex patients.
• Provide gait training, balance training, therapeutic exercise, transfer training, and functional mobility interventions.
• Collaborate with nursing staff, physicians, OT, SLP, and case management.
• Educate patients and caregivers on safety strategies and home exercise programs.
• Complete timely documentation and progress reporting.

Requirements

• Virginia PT License Required
• BLS Required
• SNF or post-acute experience preferred
