About the role
CoreMedical Group is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week contract starting July 10, 2026, at High Hope Care Center in Sulphur, Louisiana.
Responsibilities
- Provide physical therapy services to patients in a skilled nursing facility setting.
- Develop and implement individualized treatment plans based on patient needs.
- Assess patient conditions and progress regularly to adjust treatment plans as necessary.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Document patient care activities and progress in accordance with regulatory requirements.
Requirements
- Current licensure as a Physical Therapist in the state of Louisiana.
- At least one year of recent relevant experience in a skilled nursing facility setting preferred.
- Knowledge of Medicare/Medicaid regulations and reimbursement processes.
- Excellent communication and interpersonal skills.
Qualifications
- Master’s degree in Physical Therapy from an accredited program.
- Passing scores on the National Physical Therapy Examination (NPTE).
Skills
- Proficiency in using medical equipment and technology.
- Strong organizational and time management skills.
- Ability to work independently and as part of a team.

Pay
The bill rate is $75 per hour, with potential for additional incentives and benefits.
Schedule
