Travel Skilled Nursing Facility (SNF) Physical Therapist - $1,940 per week

About the role

CoreMedical Group is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week contract starting July 26, 2026, in Decaturville, Tennessee.

Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility setting
• Develop and implement individualized treatment plans based on patient needs
• Evaluate patient progress and adjust treatment plans as necessary
• Document patient care activities and outcomes accurately
• Collaborate with interdisciplinary teams to ensure comprehensive patient care
• Adhere to all regulatory and accreditation standards

Requirements

• Current licensure as a Physical Therapist in the state of Tennessee
• At least one year of recent relevant experience in a skilled nursing facility setting
• Knowledge of Medicare and Medicaid regulations
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team

Qualifications

• Master’s degree in Physical Therapy from an accredited program
• Passing scores on the National Physical Therapy Examination (NPTE)
• Current CPR certification

• Excellent organizational and time management skills
• Proficiency in electronic medical records systems (EMR) such as Nethealth/PCC
