About the role
Pioneer Healthcare Services is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week assignment in Lake Worth Beach, Florida. This on-site position offers day shifts of 8 hours per week, totaling 40 hours per week.
Responsibilities
- Assess patients' physical abilities and develop personalized treatment plans
- Provide therapy to improve mobility, strength, and functional independence
- Collaborate with healthcare teams to ensure comprehensive patient care
- Educate patients and families on exercise programs and injury prevention
- Monitor and document patient progress and adjust treatment plans as needed
- Implement interventions in various settings
- Participate in discharge planning and coordinate follow-up care for continued rehabilitation
Requirements
- Doctorate in Physical Therapy
- Florida Physical Therapist (PT) license
- Current BLS/CPR Certification
Pay
$1,848.93 - $1,906.11 per week
