Host Healthcare is seeking a travel nurse RN Case Manager, Acute Care Case Management for a 13-week assignment starting July 27, 2026, in Oakland, California.

Responsibilities

• Assess patient needs and develop care plans
• Coordinate care among various healthcare providers
• Manage patient transitions between facilities
• Ensure compliance with regulatory standards
• Communicate effectively with patients, families, and interdisciplinary teams

Requirements

• Bachelor's degree in Nursing (BSN)
• Current California RN license
• Minimum 1 year of acute care case management experience
• Experience with electronic health records (EHRs)
• Strong communication and problem-solving skills

Pay
