RN Case Manager position at Massachusetts General Hospital, part of Mass General Brigham system in Boston, Massachusetts.

Role involves case management and discharge planning.
Job Requirements
- Primary Source Verification of professional license and/or certification required within 30 days of start date; MA Conditional Licenses not accepted; both NURSYS and BON required for nursing licenses.
- Documentation of two measles, two mumps, and one rubella vaccine or two MMR vaccines, or proof of immunity via blood test.
- Documentation of TB skin test screening within 3 months of start date or negative QuantiFERON TB Test or T-spot within 3 months of start date.
- Physician Health Statement signed by MD/DO/NP/PA within 365 days of start date.
- MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break.
- MGB Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break.
- MGB Resume including past 7 years of work experience with city and state.
- MGB Skills Checklist completed within 1 year of submission.
- MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions.
- MGB Education Verification through background check company.
- MGB SSN Trace with addresses/aliases for past 7 years.
- MGB National Sex Offender Check within 30 days of start and updated annually.
- MGB Exclusion Checks within 30 days of start and annually.
- Fit Test (N95) within 365 days to start date and updated annually; no exemptions allowed.
- Respirator Medical Clearance Attestation required if fit test is required for skill.
Schedule Information
- 40 hours per week.
- Day shift, Monday through Friday.
- Shift hours from 07:30 to 18:00.
Additional Details
- No religious or medical exemptions allowed for N95 Fit Test.
- Fit Test updates can be done at Martha's Vineyard & Nantucket if traveler extends beyond original assignment due to limited resources.
- Positive PPD requires negative chest x-ray documentation (optional).
- Tdap vaccine within 10 years preferred but optional.
- Name change documentation required if credentials do not match current legal name or profile.
- TB Symptom Survey required if positive PPD, updated annually (optional).
- Candidate Submission Coversheet required for submission to initial assignment and new facility.
