About the role
Access Healthcare is seeking a travel nurse RN Acute Care Case Management for a 13-week assignment in Appleton, Wisconsin.
Responsibilities
- Provide case management services to patients in an acute care setting.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
- Assess patient needs and coordinate referrals as necessary.
- Ensure timely and accurate documentation of patient care activities.
- Participate in quality improvement initiatives as required.
Requirements
- Current RN license in good standing.
- At least 1 year of acute care case management experience.
- Experience with inpatient hospital settings.
- Ability to float to Mercy and/or Calumet if needed.
Qualifications
- Must be willing to work 40 hours per week, 8-hour shifts.
- Must have a valid driver’s license and reliable transportation.
- Must be able to pass a background check and drug screening.
Skills
- Strong communication skills.
- Excellent organizational and time management skills.
- Proficient in electronic health record systems.
Benefits
Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
