Host Healthcare is seeking a travel nurse RN Case Manager, Acute Care Case Management for a 13-week assignment starting July 27, 2026, in Crescent City, California.

Responsibilities

• Manage patient care plans and coordinate services across various departments
• Assess patient needs and develop care plans
• Communicate with physicians, other healthcare providers, and patients to ensure effective care coordination
• Ensure compliance with regulatory standards and policies
• Document patient interactions and progress in medical records

Requirements

• Bachelor's degree in Nursing (BSN)
• Current California RN license
• Minimum 1 year of acute care case management experience
• Experience with electronic health records (EHRs)
• Strong communication and organizational skills

Qualifications

• Passing score on the NCLEX-RN exam
• Knowledge of hospital policies and procedures
• Ability to work independently and as part of a team
