About the role
Care Team Solutions is seeking a travel nurse RN Acute Care Case Management for a 13-week assignment starting ASAP in Wichita, Kansas.
Responsibilities
- Manage patient care plans and coordinate services
- Collaborate with interdisciplinary teams to ensure patient needs are met
- Assess patients' health status and develop treatment plans
- Document patient progress and communicate with healthcare providers
Requirements
- 2+ years of recent Case Manager experience in acute care settings
- Current RN license in good standing
- Knowledge of case management principles and practices
Qualifications
- Passport required for international travel
- Valid driver’s license and reliable transportation
Skills
- Strong communication and interpersonal skills
- Excellent organizational and time management skills
- Ability to work independently and as part of a team
