Responsibilities
- Manage patient cases in an acute care setting
- Focus on discharge planning and coordination of care
- Collaborate with interdisciplinary teams (IDT)
- Ensure compliance with hospital policies and regulations
Requirements
- Minimum 5 years recent RN Case Management experience
- Experience in LTAC or STACH settings
- Experience with MCG and discharge planning across acute care settings
- Strong collaboration skills within IDT
- Current BLS certification (through the American Heart Association)
- Active Texas RN or multi-state RN license
- Work schedule: Monday - Friday, 8:00 AM - 4:30 PM
