Responsibilities
- Manage patient care coordination and discharge planning
- Collaborate with interdisciplinary team to ensure patient needs are met
- Assess patient needs and develop care plans
- Provide education to patients and families
Requirements
- Current New York license
- AHA BLS and ACLS certification
- Additional certifications related to this specialty
- 2 years of recent experience in Case Manager
Qualifications
- Primary residence must be at least 50 miles from the facility address
- Shift: 8 hours, days
