Care Team Solutions is seeking a travel nurse RN Acute Care Case Management for a 13-week assignment starting July 13, 2026 in Portsmouth, New Hampshire.

Responsibilities

• Manage patient care plans and coordinate care with interdisciplinary teams
• Assess patient needs and develop care plans
• Collaborate with physicians and other healthcare providers
• Ensure compliance with regulatory standards and policies

Requirements

• At least 2+ years of recent acute care case management experience
• Current RN license in good standing
• Knowledge of relevant healthcare regulations and standards

Qualifications

• Strong communication and interpersonal skills
• Ability to manage multiple tasks and prioritize effectively
• Proficiency in electronic health records (EHR)
